Holiday Hours:

Thanksgiving- The office will be closed on Thanksgiving in observance of the holiday.

Christmas- The office will be closed on Christmas Eve and Christmas Day in observance of the holiday.

2016 Holiday Giving

We are pleased to continue our annual holiday tradition of collecting donations for those in need. Throughout DECEMBER we will be accepting donations of the following items:

  • Paper goods (plates and napkins) and plastic utensils (forks and spoons)
  • Canned goods (e.g., fruits and vegetables)
  • Other non-perishable food items (e.g., pasta and dry food products)

Your contributions are greatly appreciated! Donations will be delivered to The Oak Park/River Forest Food Pantry.

Receive 1 Raffle entry ticket for every item you donate. The winner of the raffle will receive a prize!

Please pay your office visit co-pay at the time of service.

The cancellation fee is now $50 effective 5/2/2016.

Please provide us with any updated insurance and/or credit card information if needed.

Please notify the front office if your card on file needs to be updated as a result of it being lost, stolen or replaced.

Please notify the front office on address changes.
